Disclosing disability in personal statement?

About to apply for a masters degree in physics. I have a strong profile, but my grades are below what top programs would expect and my advisor says I should explain that in my personal statement. I don't want to make excuses, but I really did have several extenuating circumstances. Notably, I get academic accommodations for a disability that I was only diagnosed with halfway through my degree. Should I mention this? Also, should I be vague about the nature of my disability like I am here, or should I actually go into more detail-I want to give some details but worry that that could hurt me.
