The gGRE is good, you may want to retake pGRE to attempt a 980/990 for extra security and feeling good. (I did get 900 on my 2nd try and 980 on my third, even that didn’t help me).

Your home institution is ivy, you say.
I think you should apply to your home institution (this should be your safety, as this is where you have done most of your research exp, and where the LoRs are most effective to), and some other schools that are in the same/higher tier as your institutions.

I believe you can look at the past applicant profiles posts in this forum to find which other schools that your school is usually grouped with in the Reach/Super Reach/ Match group.

I do not think you will need to apply to lower tier schools in the US for safety. Your home institution should accept you because they have all your data and achievements.

You can also consider Japan for neutrino, but language might be the extra barrier to break...

As usual, I second Nishikata with one caveat. Apply to one other safety school other, than your home uni (awesome that you have an Ivy home uni— cherish this). There are schools in the top 50 (based on your info, I'd recommend #20-#50 depending on how much courage you have —— or apply to two other safeties, one around #20 and around around #40) that have BRILLIANT professors in lattice QFT.

Step #3: You got lucky with HEP. Use InspireHEP (alternatively arXiv, Google Scholar, Research Gate) to see if they are activing persuing research you think is cool. Keep an eye on the publications in recent years. Linearly decreasing (if you see exponentially decreased, move on, they are retiring/leaving) may be a bad — http://inspirehep.net/
